Abdominal aortic aneurysm (AAA) is the thirteenth-leading cause of death in the U.S. and a leading cause of death for American men over the age of 65. The most common location for aortic aneurysms is in the abdominal aorta. The rest occur in the thoracic area. As a result, most AAAs are located below the level of the renal arteries, and approximately 20 to 30% extend into one or both of the iliac arteries. When blood vessels cannot be repaired endovascularly, open surgical repair (OSR), or bypass, is the only option. In cases when a native vessel cannot be used, artificial grafts are employed. Artificial surgical grafts are typically manufactured using polyester or polytetrafluoroethylene (PTFE). These surgical grafts are surgically mated to the damaged vessel. Grafts can be as simple as a straight tube or can have bifurcations and/or side-branches. The market for surgical grafts in the peripheral vasculature includes the following segments, each discussed in detail later in this chapter: lower-limb vessel bypass (above-the-knee and below-the-knee indications), extra-anatomical grafts and aortic aneurysm repair. Lower-limb bypass grafts are generally of small diameter and tend to be made of expanded PTFE (ePTFE) due to the material’s greater flexibility and improved patency. In contrast, aortic aneurysm and extra-anatomical grafts are of larger diameter and are dominated by polyester devices. Polyester has the advantage of improved handling characteristics. The total market for stent grafts comprises the abdominal aortic aneurysm (AAA) stent graft segment and the thoracic aortic aneurysm (TAA) stent graft segment. The value of the AAA stent graft segment exceeded the value of the TAA stent graft segment in 2015. Growth in the value of the total market for stent grafts will be driven by growth in the unit sales of both the AAA and TAA stent graft segments. Growth in the value of the TAA segment is expected to outpace growth in the value of the AAA segment throughout the forecast period. The U.S. surgical graft market comprises the lower limb surgical graft market, the aorta repair surgical graft market and the extra-anatomical bypass surgical graft market. The lower limb surgical graft market is the largest segment, representing over 60% of the total surgical graft market in 2015.
